Compare a current draft or issued separation packet with common DoD field instructions. Review the DD Form 214 first, then review DD Form 214-1 only if that addendum is included in the packet.

Current form only. This checklist covers the DD Form 214-1 issued under the same DoD instruction. Older printings and service-specific layers are out of scope.

The DD Form 214-1 is the Reserve Component Addendum of the DD Form 214 series. It is issued only with a DD Form 214, it is not required with every DD Form 214, and it carries the same serial number as its DD Form 214. Review this section whenever the separation packet includes the addendum. The checklist never asks whether the packet includes one.
